I'm in a swap right now where both I and my swap partner confirmed the swap almost simultaneously (within a minute). The "event log" in the swap shows both of us confirming the swap, but at least from my side the website only shows the swap as confirmed by me.
This seems like a bug. I'm still waiting to hear from him as to whether or not he still has the ability to confirm the swap. If not, there's no way for us to rate the swap. Even if he can confirm again, this doesn't seem like expected behavior when both parties click confirm at close to the same time. Shouldn't the second one's confirmation approve the first party's offer?
Further info: my swap partner was able to confirm the swap, so we will be able to to rate it.
Looks like what happened was:
A) Confirms swap.
B) Was looking at previous page and confirms the swap. This "cancels" A's confirmation and creates a confirmation offer with what was on B's page.
A) Confirms B's offer.
So I guess this is just basically a web concurrency problem, where I was viewing an out of date version of the swap page and confirmed it. Not sure whether it's something that needs addressing, since it eventually worked out.
