I just can't understand how it's possible that an existing KM number can be requested and, even worse, get accepted, after I went through the whole issuer to make sure every one is listed properly.
Crazy idea: when cleaning up stuff like this, ALWAYS document the referee who accepted the duplicate and be surprised how often the same names will show up…
